If it seems like the GU0 record for medical billing of claims is endless, well, it is close to it. The CMN itself has over 70 fields. The majority of them are so cryptically mapped that it is impossible for a biller to understand one field from another without going through the manuals, which usually aren't much help anyway. In this installment we'll be continuing with our endless revue of the GU0 record, picking up with field number 46.

GU0 field 46, position 132, is Reply ALN L01 N21. This is the response to the twenty-first question on any DMERC certification requiring a one position response. All forms for this question are reserved for future use. This field covers all generic CMNs.

GU0 field 47, position 133, is Reply ALN L01 N22. This is the response to the twenty-second question on any DMERC certification requiring a one position response. All forms for this question are reserved for future use. This field covers all generic CMNs.

GU0 field 48, position 134, is Reply ALN L01 N23. This is the response to the twenty-third question on any DMERC certification requiring a one position response. All forms for this question are reserved for future use. This field covers all generic CMNs.

GU0 field 49, position 135, is Reply ALN L01 N24. This is the response to the twenty-fourth question on any DMERC certification requiring a one position response. All forms for this question are reserved for future use. This field covers all generic CMNs.

GU0 field 50, positions 136 - 140, is Reply ALN L05 N01. This is the response to the first question on any DMERC certification requiring a five position response. Form 09 is used here to enter in the HCPCS code if the CMN itself is for some kind of a pump. Form 08 is reserved for future use.

GU0 field 51, positions 141 - 145, is Reply ALN L05 N02. This is the response to the second question on any DMERC certification requiring a five position response. Form 08 is used here to enter in a US postal zip code. This has to be the zip code from the state where the operation was performed.

GU0 field 52, positions 146 - 150, is Reply ALN L05 N03. This is the response to the third question on any DMERC certification requiring a five position response. All forms for this field are reserved for future use. This applies to all CMNs.

GU0 field 53, positions 151 - 158, is Reply ALN L08 N01. This is the response to the first question on any DMERC certification requiring an eight position response. This is one of the most confusing fields in the GU0 record, so let's start with the easy qualifications first. Forms 05 and 08 are reserved for future use. Form 06 requires a date of surgery entry ONLY if GU0 field 26 is N or D. Also, this has to be surgery that resulted in acute post operative pain. Now do you understand why medical billers go crazy?
